Welcome to the physical therapy facility of Keith Golka, PT

To make an appointment or to ask us a question, simply call our office at (631) 567-6421. We are located in the lovely town of Sayville at 291 C West Main Street

We accept most all major insurance plans, Worker’s Compensation and No Fault plans. If we are not listed as a physical therapy provider in your insurance directory please call us to inquire. We look forward to initiating a treatment program for you.At your initial evaluation appointment, you will need the following:

• A prescription from a medical doctor
• An insurance referral may also be required by your insurance plan in addition to a prescription. We recommend calling the member services department of your insurance company to verify your physical therapy benefit. Ask about copayments, coinsurance, deductibles, number of visits allowed, etc.
• Your medical insurance card, list of current medications, and any diagnostic test results (MRI, x-ray, CAT scan, blood work, etc.)
• Copayment if required Note: it is illegal for us, as contracted providers with your health insurance plan, to waive any copayments or deductibles
• Please try to arrive 10-15 minutes prior to your scheduled appointment time to allow adequate time to fill out necessary paperwork.
• Wear comfortable, loose fitting clothes that will enable us to examine and treat you

The initial evaluation and treatment will last approximately 60-75 minutes

There may be additional paperwork required with your insurance plan that will be available at the time of your initial visit
